Patients Without Health Insurance
For patients who do not have health insurance, payment is due at the time of service. Note: All new patients below the age of 18, must be accompanied by an adult for the first visit. Minors can be seen without adult supervision after the first visit only if the patient's guardian fills out and signs a minor consent form.
